Assumption of Risk and Release
By signing this Agreement, I acknowledge the contagious nature of COVID-19 and I voluntarily and knowingly assume the risk that I may be exposed to and become infected with COVID-19.
If I am exposed to COVID-19, by attending this shoot, the exposure to infection could cause me personal injury, illness, permanent disability, and death. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury, illness, disability, death, damage, loss, and expense of any kind that I may experience or incur in connection with my attendance at and participation in this shoot.
I hereby release, discharge, and hold harmless Photogen Inc. / dba Rivets and Roses, its owners, shareholders, employees, agents, and representatives from all claims arising out of my attending and participating in this shoot, including all damages, costs, and expenses of any kind. I further agree to alert Photogen Inc., and/or Eliesa Johnson if I am diagnosed with COVID-19 within 15 days after attendance and participation in this shoot.
